Losing the original Microsoft 365 administrator does not necessarily mean losing the tenant, but recovery is easier when the clinic can prove ownership of its domain, billing relationship, and organization.

Start by identifying what the clinic still controls

  • Confirm access to the domain registrar and DNS provider.
  • Locate Microsoft billing records, subscription invoices, tenant names, and prior administrator usernames.
  • Identify any remaining Global Administrator, billing administrator, partner, or reseller access.
  • Preserve current email and user access while recovery is underway.

Use the correct recovery path

  • Try normal self-service account recovery only when recovery methods are clinic-controlled.
  • Open a Microsoft support case when no functioning administrator remains.
  • Be prepared to add or verify a DNS record to demonstrate control of the clinic domain.
  • Document every case number, support contact, requested proof, and completed change.

After access is restored

  • Create at least two clinic-controlled administrative paths.
  • Separate daily administration from emergency access.
  • Review roles, MFA methods, partner relationships, subscriptions, and billing contacts.
  • Test the recovery accounts and record the result in the clinic IT handbook.

Practical note: Technology, licensing, vendor capabilities, and regulatory obligations vary. Confirm the clinic’s current configuration before making changes.

What recovery usually requires

Recovery may require proving control of the clinic's domain, identifying billing or subscription information, providing business documentation, and working through Microsoft support. The exact path depends on whether another administrator, reseller, partner, or verified domain remains available.

Preserve evidence

Do not delete DNS records, cancel subscriptions, or replace payment methods before documenting the tenant name, domains, invoices, support case numbers, affected users, and any remaining sign-in access.

After access is restored

Create at least two clinic-controlled administrators, replace personal recovery methods, review roles and applications, export important configuration, test emergency access, and document the support and ownership history.
